I. Fill out the
Registration Form and
send to DHQ by June 15th, 2010.
II. Fill out all
forms that apply to you and mail or fax them to THQ
by
July 20th, 2010.
a. Everyone must
fill out the
Authorization to Administer Medications
form.
There is a release form for those who are
18 years and older
and it does not need a parent/guardian signature.
There is also a separate form for those who are
17 years old and younger, which needs to be signed by a
parent/guardian and includes getting a Physical.
*Please note:
After sending in a copy of
the medical release form to THQ, delegates must also
BRING THE ORIGINAL FORM WITH
THEM WHILE TRAVELING TO CAMP.
This is needed while traveling in case there is a
need for medical treatment. Also, if you are
at another camp during the summer and already have a
completed health form, this can be used but YOU are
responsible for providing a copy of the form to THQ.
b.
The Authorization
to
Administer Medication From only needs to
be filled out if you are bringing medication to camp
with you. This includes prescription and any over
the counter mediations, for all ages. All
medications need to be checked in with the nurse at
registration.
c.
The Photography
Release Form must be filled out by everyone. If you
are 18 and older, you sign the form yourself. If
you are 17 and younger, a parent/guardian must sign
it. This form grants permission to take your
picture at TYI.
d.
The
Do's and Don'ts form
must be
filled out by everyone. It does not require a
parent/guardian signature, only the delegate’s
signature.
III.
Complete the
Elective Class Sign up Form.
You will be making your
selection of Elective Classes. All classes have
a cut off point, so in order
to get the class you want – sign up as soon as
possible! This form is due
to THQ no later than July 20th, 2010.
If we do not receive a sign up form,
we will have to assign you to a class.
